We got some from gearclub, they came out looking great. Delighted with them.
I'll second bioracer but would also suggest checking out Milltag. Pretty sure their minimum order quantity is low.
I've used a couple and always ended up going back to Primal - great service.
Our kits have had Primal logos on in a very subtle manner, I've never had an insistence we plaster their logo all over it at all. I remove most of them from the templates and resize them to fit the design.

Our club uses velotec.cc great value and spec bibs and short sleeve jerseys - based in Ireland.
Endura are too expensive and too big to care.
Primal are excellent but they insist on Primal logo's front and centre on everything unless you pay more.
Our club went with http://www.bioracer.co.uk/en/ We also tried Endura but weren’t overly impressed
Not used their custom service, but http://primaleurope.com kit is good. I believe they do the Road.cc kit.
Force GB do our club kit and are happy doing small runs without a massive premium.
We use WoW custom (http://wowcustom.co.uk/) and they're great, very good value, lots of help with the design and different jersey options (long/short sleeved, standard and race fit). It can take a while for orders to come through so be careful if time is limited, but otherwise highly recommended
