My experience of that Met Police form: I put a bit of 'close pass' footage on Twitter a while ago and tagged the Met Police in the tweet. They replied, asking me to submit it to that linked Met Police online form. I did this, including the bit where you agree to appear in court if necessary. They then asked me to delete all other copies of the footage from YouTube, etc., which I did. Nothing happened for a bit - they then emailed me saying that they had reviewed the footage and were going to contact the driver/registered owner. I didn't hear anything after that, so I assume the driver admitted guilt. All in all, quite an easy process I thought, although not quick.
