Quite lucky where I live with the countryside going right into the city centre with old tram ways and railway lines etc...plus riverside rides free of traffic. Damned near perfect urban riding really.
Probably just as well because the roads are beyond dangerous going over the Ribble....
I'm sorry Jack but I wont be answering your survey as having read your post I do feel you have fallen into a very big hole once you start saying that you want to make cycling in the city safer. I do live in London and commuted long before cycling became sexy but have never considered cycling unsafe. Like all other activities there are risk elements to be taken into account not danger. Would you say driving or being a pedestrian required the city to be made less dangerous, I very much doubt it!!
I figured that "fresh air" was relative.
"Fresh Air" in a city whilst commuting, not only isn't this really true it's too close to one of the other options.
Also define 'gadget'.
When asking what would you invest in if you don't already use/posses you could have a 'none' option.
Interesting that you put helmet as the first option in your list of things you use, why was that, it's certainly not the first thing I would think about as a road cyclist in urban cycling nor even safety, you missed gloves and glasses, far more important from a safety POV for urban riding.
A question you could have asked regarding safety is do you do safety checks you bike before going out i.e. brakes/tyres/loose fittings/bolts etc and how often do you do this.
