Cable to hydraulic disc brakes.

Is it possible to retrofit a hydraulic system to replace a cable disc brake set up? The bike is a 2016 Cannondale Synapse 105 Disc so I just want to have a look at the possibility. I am in Stroud so there are loads of hills and coming down can be a bit nerve racking without being able to stop well!
